CHLEBAK, Rose - It is with sad hearts we say goodbye to our mother passed away peacefully on May 25, 2019 in her 98th year.
She is predeceased by her beloved husband, Joe, sons Ron and Ken, and siblings Bill, John, Helen and Mike.
She is survived and will be sadly missed by son Al, daughter-in-law Pam, and grandchildren Ryan, Nathan and Ashley.
We sincerely thank the staff of Orchard Villa Long Term Care for the many years of dedicated care and kindness to our mom.
We take some comfort in knowing that she is now reunited with Dad and the whole gang of family and friends.
Visitation will be held at Heritage Funeral Centre, Toronto on Wednesday, May 29th, 2019 from 7:00pm - 9:00pm. The service will take place at Canadian Martyrs Catholic Church, 522 Plains Rd, Toronto on Thursday, May 30th, 2019 st 11:00am.
